Thank you for your interest in our Mass enrollment cards. Please look through the various cards that we offer. Then choose the card(s) you would like by filling in the quantity you desire in the appropriate box next to each card's picture. Next, provide your mailing information at the bottom of the order form, and press the "Send Order" button.

Your cards will be sent by mail to the address that you provide on this order form.

Perpetual Enrollment Certificate

The next item is an engraved Certificate of Perpetual Enrollment and comes in a padded folder-type frame with a choice of three different color prints.

Each certificate comes inscribed with your name and the name of the person being enrolled. So when you order, please specify the names of the persons being enrolled so that the certificates can be prepared. Also tell us how you want your name to appear on the certificate. If you order more than one, the certificates and color prints are interchangeable so that you can decide later which print you want to use for each person you enroll.
